Below are templates of documents and surveys I use in my courses. 

Course Schedule

This is the Course Schedule template that follows QM and OSCQR Rubric Guidelines. 

Course Blueprint

This serves as an example of a Course Blueprint, which I use to detail the structure of my courses. It contains a Course Alignment Table, demonstrating how the course outcomes correlate with Chapter Objectives, Learning Materials, Assignments, and Assessments.

Assignment Template | Sample

I used this general Assignment Template for each assignment. It is based on the TILT strategy (Transparency in Learning and Teaching). The sample was used as part of the 4-week project in CIS 110.


Weekly Overview | Sample


This is an example of a Weekly Overview Document that guides students through each week's learning process. I think the focus should be on the content, not the format, as the former can be adjusted according to your teaching style. I have included a sample of the one I used in my CIS 110 to guide students through the learning process for the week.

Course Check-In


This is a sample of the Course Check-in included with every course. In my courses, I like to check in about three weeks to ensure that students are comfortable with the course and understand the expectations for the rest of the semester. This check-in also allows students to express concerns or ask questions about completing their work.

Mid-Semester Survey


This is an assessment of the course. Students can provide feedback to the instructor, highlighting what is effective and suggesting areas for improvement.

Sample Template Course in Moodle 

This is a sample course layout in Moodle. The Welcome and Onboarding sections may be designed by the schools' Online or Distance Learning departments, but the course includes some sample information. 

Week 2, provides a Sample Layout of a weekly layout. The KWL chart can be used as a tool for students to track learning. The exercises provide ideas of what could be included weekly as a learning path design idea. The muddiest points forum is an optional assignment that can provide instructors with feedback for areas of knowledge gaps.
